At True North Clinic, our Continued Care services are designed to help individuals maintain their progress, build a stable life in recovery, and stay connected to the support they need long after completing intensive treatment.
What Is Continued Care?
Continued Care—sometimes referred to as aftercare or step-down support—is a structured plan that bridges the gap between treatment and everyday life. It’s personalized to each individual’s needs and may include a combination of ongoing therapy, recovery coaching, and resource coordination.
Our Continued Care services help clients:

Maintain momentum in their recovery journey
Continued Care typically begins after completing services like residential treatment, PHP, or IOP. Together with your care team, you’ll develop a tailored plan that may include:

Weekly or bi-weekly therapy sessions

Recovery coaching

Ongoing case management

Alumni meetings and sober social activities

Referrals to community-based services and support groups
This flexible structure allows individuals to continue progressing in recovery while rebuilding their personal, family, and work lives.

Statewide Virtual Services
In addition to in-person services we offer at True North Clinic, we also continue to provide services virtually, also known as telepsychiatry and telehealth services. These cutting-edge healthcare practices enable assessment and treatment through technology, typically by way of video conferencing. This approach opens up treatment opportunities to a wider audience, eliminating barriers like transportation or geographical constraints.
