At True North Clinic, we believe recovery is not a one-size-fits-all journey. That’s why we offer a tiered, individualized approach known as the continuum of care. This structured progression of support helps clients move through each stage of recovery with the right level of care, exactly when they need it. From high-intensity support to long-term maintenance, here’s how our continuum of care empowers lasting healing.


What Is the Continuum of Care?

The continuum of care refers to a step-by-step treatment model that adapts to each individual’s needs, ensuring they receive the appropriate level of clinical, emotional, and peer support at every stage of their recovery. It’s designed to meet clients where they are—whether they’re just beginning detox or stepping down into independent life.


At True North Clinic, our tiered system includes:


For clients beginning recovery who need a safe and medically monitored withdrawal.

Detox is often the first step in the continuum of care. Our outpatient detox program provides medical supervision and clinical support to ensure clients withdraw from substances safely and with dignity. This level of care allows individuals to begin treatment while remaining connected to daily responsibilities, when appropriate.


A structured, supportive environment for those needing stabilization and intensive care.

Our Level 3.1 Residential program is ideal for clients who need more structure than outpatient care can provide but do not require 24/7 medical supervision. In this phase, clients live onsite and participate in daily therapeutic programming while building the foundational skills for sobriety. This stage emphasizes routine, community, and individualized care planning.


Daytime treatment with clinical intensity—without overnight stays.

As clients progress, they may transition into our Partial Hospitalization Program. PHP includes full-day programming (typically 5–6 hours/day) with therapy, psychoeducation, group support, and medical monitoring. Clients return home or to sober living in the evenings, allowing them to practice recovery in real-world settings with continued support.


Flexible therapy for those balancing recovery with work, school, or family life.

IOP is a step down from PHP, offering fewer weekly hours but still robust therapeutic engagement. True North’s IOP helps clients solidify coping strategies, strengthen relapse prevention skills, and maintain accountability while reintegrating into daily life. It’s designed to fit into schedules while continuing to address emotional and behavioral health.


Ongoing connection for sustained recovery and community.

Recovery doesn’t end when formal treatment does. Our Continued Care program provides long-term support through alumni groups, peer check-ins, and access to counseling as needed. This level fosters belonging, reduces isolation, and supports clients as they navigate life post-treatment.
